JUMP FOR JOY- RENTAL AGREEMENT CUSTOMER: RENTAL DATE: ADDRESS: TIME START: TIME END: CITY: CHARACTER: PHONE: RENTAL FEE: Delivery: To the address specified by the lessee (customer). Lessee grants JUMP FOR JOY the right to enter said property for the delivery and return of Jump For Joy unit at approximate specified time. AGREEMENT & GENERAL RULES TO FOLLOW DURING USE OF JUMP FOR JOY UNIT ARE: 1. Compatible age groups must play on the unit at the same time. Children in the unit at the same time should be close in weight. The maximum number of jumpers of each group that should play on the unit at one time is as follows: AGE GROUP: under 8 years 9-12 years older teens /adults # OF JUMPERS: 8 6 4 2. REMOVE SHOES BEFORE USING UNIT. 3. TO AVOID NECK AND BACK INJURIES, FLIPS ARE NOT ALLOWED. 4. CHILDREN’S SAFETY DEPENDS UPON YOU AND SUPERVISION IS REQUIRED. AS LESSEE OF SAID JUMP FOR JOY PROPERTY, THE SAFETY OF ALL WHO ARE USING SAID PROPERTY IS YOUR RESPONSIBILITY ! Agreed to by: Ö___________ 5. NO SILLY STRING, GUM, CANDY, FOOD, DRINKS, ICE CREAM, INK, PAINT, CONFETTI allowed in or on unit. TURN SPRINKLER SYSTEM OFF ! NO WATER ALLOWED ! If upon pick-up, cleaning is required, lessee agrees to pay a MINIMUM $100.00 cleaning fee. Agreed to by Ö_____________ 6. Do not move Jump For Joy unit from place where installed. If it moves, pull corner back to original location. 7. NO REFUNDS: Once equipment is delivered payment in full must be made and will not be refunded due to bad weather or any other circumstances. 8. PROBLEMS: Equipment is reliable but should the jump begin to deflate check the following; (A) Motor has stopped - Check cord connection at outlet and near motor. DO NOT USE YOUR OWN CORDS. No more than 100 feet of cord and NOTHING ELSE ON OUTLET. Stronger outlets are in kitchen & laundry rooms. (B) Motor continues to run - Check air intake on side of motor for blockage. Check both tubes on back of unit for snugness and retie if necessary. (C) LESSOR IS NOT RESPONSIBLE FOR EQUIPMENT FAILURE DUE TO LESSEE’S ELECTRICITY, IE. INSUFFICIENT POWER, CIRCUIT BREAKERS, OR WIRING. MOTOR CAN USE UP TO 15 AMPS of ELECTRICITY REMEMBER NOTHING ELSE BESIDES MOTOR ON OUTLET. (D) IF THERE’S A PROBLEM WITH THE UNIT ON SITE THAT CANNOT BE CORRECTED, CALL: (310) 316-0158 9. SAFE OPERATION & DAMAGE RESPONSIBILITY ACKNOWLEDGMENT: LESSEE ACKNOWLEDGES THAT THEY HAVE BEEN INSTRUCTED ABOUT AND FULLY UNDERSTAND THE SAFE OPERATION OF THE JUMP FOR JOY UNIT THAT IS THE SUBJECT OF THIS RENTAL AGREEMENT AND LESSEE AGREES TO OBSERVE ALL SAFETY PRECAUTIONS. LESSEE ACKNOWLEDGES THAT THEY HAVE INSPECTED THE JUMP FOR JOY UNIT AND THAT THEY ARE RECEIVING THIS UNIT UNDAMAGED UNLESS NOTED AND IN FULL WORKING ORDER. LESSEE AGREES TO PAY LESSOR, ON RETURN OF THE JUMP UNIT, FOR ALL CHARGES INCIDENTAL TO BREAKAGE OR DAMAGE, ORDINARY WEAR EXCEPTED, TO JUMP FOR JOY UNIT DURING THE TERM OF THIS RENTAL AGREEMENT. BY INITIALING, LESSEE AGREES TO THIS PROVISION. Ö________________ 10. JUMP FOR JOY RELEASE OF LIABILITY - Please read carefully ! TERMS OF LEASE: The lessee shall be in charge of operation and is fully responsible for the operation after receiving unit, as well as the return of the unit undamaged and in working order. JUMP FOR JOY is not responsible for injury occurring to Lessee or to any other persons using the leased property and Lessee further agrees to hold JUMP FOR JOY free and harmless against any injury claims. The Lessee shall indemnify JUMP FOR JOY for any costs incurred due to claims from anyone and for any attorney fees and related costs involving the use and return of the leased products should legal action become necessary.
